New Patients



We have taken the difficult decision to temporarily close our patient list to those wishing to register with the practice.


Patient safety is of paramount importance and we believe, currently, that additional patients would jeopardise our ability to provide safe care for our registered patients, or to carry out contractual obligations to meet our patients' core clinical needs. 


This will not include first-degree relatives in the same household of patients currently registered with the practice, or newborn babies of registered mothers.

To register as a new patient at this practice you will need to complete a GMS1 form titled “Family doctor services registration”, which is available from Reception.  Together with this form we ask that you provide us with some basic health information details on the New Patient Registration Form, also available from Reception.  Once these documents have been completed, please return to the surgery and make an appointment for a new patient registration check with the nurse.

Each new patient will be assigned an accountable GP.  In order to maintain continuity of treatment we encourage our patients to remain with the same doctor (patients have a right to express a preference of practitioner and whilst the practice will endeavour to comply it might not always be possible).

You can book appointments and order your repeat prescriptions online.  It is quick and easy to register for Online Services - a member of the practice team will provide you with your username and password when you complete Patient Online Registration Form.  Please see our patient guide to online services for more information.
